How All-On-4 works

Four implants, one fixed arch


All-On-4 replaces an entire arch of teeth using four strategically positioned implants rather than one implant per tooth. The rear implants are angled, which lets them engage denser bone toward the front of the jaw and frequently avoids the need for extensive grafting in the back.

The procedure involves removing any remaining teeth, placing the implants, and securing a fixed, functional arch over the course of a single day. You do not leave without teeth.

The temporary arch stays in place while the bone integrates over the following months, after which the permanent zirconia arch is fitted. Unlike a denture, it is fixed. It does not come out, it does not move while you eat, and it is not held in by suction or adhesive.

Candidacy

Who All-On-4 is actually for


  • You have lost most or all of the teeth in an arch, or the remaining ones are failing and not worth saving individually
  • You already wear a denture and dislike the movement, the adhesive, or what it has done to what you can eat
  • You have been quoted individual implants for every missing tooth and the total is out of reach
  • You have been told there is not enough bone for conventional implants, since the angled rear placement in All-On-4 is designed specifically to work around that
  • You want the result fixed in place rather than something removed at night
  • You are able to attend follow-up appointments during the integration period

The day itself

What Teeth in a Day actually involves


One appointment, several hours, and you leave with fixed teeth in place.

Planning

Imaging determines bone volume and where the four implants can be angled to find the densest bone. This is done before the day, not on it.

Extractions

Any remaining teeth in the arch are removed. This happens under the same anaesthetic, so it is not a separate ordeal on a separate day.

Implants placed

Four implants are positioned, the rear pair angled to engage bone toward the front of the jaw where it is typically denser.

Temporary arch fitted

A fixed provisional arch is secured the same day. You leave with teeth, not a gap and not a plate.

Afterwards

The months between temporary and permanent


The temporary arch is fixed, functional and looks like teeth, but it is not the final result. It is deliberately made in a material that can be adjusted as the tissue settles, because the gum reshapes considerably during healing.

During this period you eat a softer diet at first and build back up. Most patients describe the first fortnight as the adjustment, then it stops being something they think about. You will have review appointments so the fit can be checked as the swelling reduces.

Once the implants have integrated with the bone, usually after several months, the permanent zirconia arch is made and fitted. Zirconia is used because it is strong enough for full chewing force and does not stain the way acrylic does.

Living with it

An All-On-4 arch does not come out. You clean it in the mouth rather than in a glass, using a water flosser and interdental brushes to keep the area underneath the arch clear. That cleaning routine is the single biggest factor in how long the result lasts, and we will show you exactly how to do it before you leave.

You will still need regular checkups. Implants do not decay, but the gum and bone around them can develop the same inflammatory problems that affect natural teeth, and that is what causes implant failure years down the line.

What is the difference between single arch and double arch?

Single arch at $20,000 restores either your upper or lower teeth. Double arch at $40,000 restores both. Which you need depends on how many teeth remain and their condition in each arch, which Dr. Mathew will assess with imaging.

Do I really get teeth the same day?

Yes, a fixed temporary arch is placed at the surgical appointment, which is why it is called Teeth in a Day. The permanent zirconia arch comes later, after the implants have integrated with the bone over several months.

Is All-On-4 the same as dentures?

No. A conventional denture rests on the gum and is removed for cleaning. An All-On-4 arch is fixed to implants, does not come out, does not move while eating, and does not require adhesive. Chewing function is far closer to natural teeth. See dentures for the removable options.

Am I a candidate if I have been told I lack bone?

Possibly. The angled placement of the rear implants in All-On-4 is designed specifically to use available bone more efficiently and often avoids the extensive grafting that conventional implants would require. Bone grafting is included in the bundled price here. Assessment with imaging gives the definite answer.

How long does the whole treatment take?

Surgery and the temporary arch happen in a single day. The permanent zirconia arch is fitted after the implants integrate, typically several months later. Between those points you have fixed teeth, not a gap.
