What an implant is
A replacement root, not just a replacement tooth
A dental implant is a small titanium post placed into the jawbone where the tooth root used to be. Over the following months the bone grows against and bonds to its surface, a process called osseointegration. That bond is what makes an implant stable enough to chew on normally.
Once integrated, an abutment is attached and a crown is fitted on top. The finished result looks and functions like a natural tooth, is cleaned like a natural tooth, and does not involve altering the teeth on either side, which is the main advantage over a bridge.
Implants also preserve bone. Jawbone maintains its volume through the stimulation of chewing forces transmitted down a root. When a tooth is lost, that stimulus disappears and the bone in that area gradually resorbs. An implant restores the stimulus.

The process
Why implants take months, and what happens in between
The appointments are short. The waiting between them is the bone doing its work.
Assessment
Examination and imaging to check bone volume and density, and to plan the position. If bone is insufficient, grafting is discussed here.
Placement
The implant is placed into the bone under local anaesthetic. Most patients report this as more comfortable than they expected, often easier than an extraction.
Integration
Three to six months while bone fuses to the implant surface. You are not without a tooth during this time; a temporary is provided where appearance matters.
Crown
An abutment is attached and the permanent crown fitted, shaped and shaded to match the teeth around it.

What makes someone a good implant candidate
- Enough healthy jawbone to hold the implant, or willingness to have a graft first
- Healthy gums, since gum disease around an implant behaves much like it does around a tooth
- Reasonable general health and healing capacity
- Not currently smoking, or willing to stop, since smoking measurably raises implant failure rates
- Diabetes that is reasonably well controlled, since blood sugar affects healing
- Willingness to maintain them, because implants still need brushing, flossing and regular checkups
Being told elsewhere that you have insufficient bone does not always end the conversation. Grafting often makes an implant possible. Ask.

Does getting an implant hurt?
The placement is done under local anaesthetic, and most patients describe it as more comfortable than an extraction. Afterwards there is typically some soreness and swelling for a few days, managed with over-the-counter pain relief. Sedation is available if the prospect makes you anxious.
How long do dental implants last?
The implant itself frequently lasts decades and often for life, because titanium integrated into bone is very stable. The crown on top is the part that wears, and may need replacing after roughly ten to fifteen years. Longevity depends substantially on gum health and not smoking.
Why does it take several months?
Because bone remodels slowly. Osseointegration, the fusing of bone to the implant surface, takes three to six months and cannot be rushed without risking failure. Loading an implant before it has integrated is the main avoidable cause of failure.
What if I have been told I do not have enough bone?
That is common, particularly if the tooth was lost years ago, because bone resorbs once the root is gone. Bone grafting can rebuild enough volume to make an implant possible in many of those cases. It adds time and cost, but it frequently changes the answer from no to yes.
Is an implant better than a bridge?
Usually, for a single missing tooth. An implant does not require cutting down the healthy teeth on either side, it preserves bone, and it is cleaned like a normal tooth. A bridge is faster and cheaper up front and remains a sensible choice in some situations, particularly where the adjacent teeth already need crowns.
Does insurance cover implants?
